Policies

We appreciate being asked to help with your child’s speech and occupational therapy needs.  We are pleased to have this opportunity to be of service and work with you.  We will do our best to earn your confidence.

Our experience is that it is worthwhile at the outset of our relationship to describe the role and responsibilities we undertake and those that we expect you to undertake.  Please let us know if you have any questions after reading this information.

 

SCOPE OF SERVICES  

We will at all times provide speech and occupational therapy services to your child to the best of our ability.  The type of services and diagnostic testing we provide will be based upon our best professional judgment and experience.  We will be happy to discuss with you additional testing or services at your request.   Any expressions on our part concerning the outcome of the therapy provided are expressions of our best professional judgment, but are not guarantees.  

Customarily, each client is served by a principal therapist.  This should be a person in whom you have confidence and with whom you enjoy working.  You are free to request a change of therapist at any time and we will do our best to honor your request. 

We will maintain any documents generated in connection with our services as required by law and our document retention policy.  We will be happy to provide you with copies of any documents in our files for the client within a reasonable time of your request and after receipt of payment for all services rendered.

 

PARENT PARTICIPATION 

Please work with your child throughout the week and between appointments with the home exercise programs we recommend and make sure your child completes any assigned homework.  Your regular assistance will increase your understanding of your child’s efforts and of the therapy treatment objectives, and will greatly promote your child’s progress.  It is your extra efforts at home that often truly make a difference. 

 

PAYMENT

Payment is due upon receipt of services and, unless otherwise agreed, is based on the amount of direct contact time with the therapist.  We accept cash, checks, MasterCard, Visa, and American Express.  We reserve the right to refuse service if your account with us is delinquent.  A schedule of our current standard rates will be made available to you.  Please ask if you have any questions about our rates.  Although it is our practice to provide as much advance notice as possible, our standard rates and terms are subject to change without notice.  In the event that you default in your payment responsibilities, you will be responsible for our costs of collection, including reasonable attorneys fees.

 

INSURANCE  

We will file claims directly for specific HMO, PPO, and POS insurance holders, and for clients with pre-approved policy coverage.  Pre-approval and referrals for services should be established prior to all visits.  In all other cases, we will invoice you directly and you will be responsible for billing your own insurance provider.  You will receive a copy of all records and charges as needed, which you may submit for direct reimbursement from your insurance company.  Insurance providers do not cover all speech/language disorders.  Please check your policy carefully.  You are responsible for any charges not covered by insurance.

 

CANCELLATIONS

Please call at least 24 hours in advance if you know that you will miss an appointment or will be late.  As openings permit, your appointment will be rescheduled.  CANCELLATIONS WITHOUT 24 HOURS ADVANCE NOTICE AND NO SHOWS WILL BE BILLED AT A RATE OF $25 PER MISSED APPOINTMENT.  If the missed appointment is rescheduled, there will be no charge.  We do understand there may be extenuating circumstances and illness and exceptions may be made on a case by case basis.

 

MISSED APPOINTMENTS

Failure to attend two consecutive appointments without notification will result in removal of subsequent appointments from our appointment schedule.  Failure to attend three consecutive appointments with notification will result in removal of subsequent appointments from our appointment schedule unless previously approved. 

 

ILLNESS

In an effort to ensure productivity for the client and the health of all patrons, a client must be free of the following symptoms for 24 hours prior to attending a therapy session:

  • Diarrhea

  • Nasal Drainage (thick mucous draining from nose)

  • Fever over 100.0 F

  • Rash (undiagnosed or contagious)

  • Vomiting

 

A client may attend a therapy session 24 hours after initial antibiotic treatment and cessation of fever for the following:

  • Strep Throat

  • Conjunctivitis ("pink eye;" white or yellow discharge from eye)

A client with chicken pox may attend a therapy session 6 days after the onset of rash and after all sores have dried and crusted.

 

PROGRESS UPDATES 

Treatment plans are regularly updated.  Following each progress report, we will discuss recent progress and goals for the coming service period.  Of course, we are available to discuss your child’s progress and our therapy programs with you at other times as well.  We want to listen to your input and be communicative with you at all times.
